Why Is It So Hard to Lose Weight? What Has Changed?
Your metabolism has changed, and that is why all the old tricks are not working. When you gain belly fat, your metabolism changes. When you have hormonal changes, your metabolism changes. As you age, your metabolism changes. Insulin resistance is one of the main causes.
What is insulin and Insulin Resistance
Insulin’s action plays a role in the body’s metabolism of carbohydrates and fat. Insulin regulates how we use and store glucose and fat. Insulin is the key to letting glucose into the cells to generate the energy required for day to day cellular function. With insulin resistance, the ‘key doesn’t fit into the lock’ and glucose can’t get into the cell. The cells are starved for fuel. So despite having plenty of sugar outside our cells, our cells send a signal to the brain to eat more sugar (NOW!!!) and so, sugar in the blood rises.
Insulin resistance (IR) is a pathological condition in which higher levels of insulin are needed in order for insulin to have its proper effects. Insulin is a growth factor and makes everything grow (including our fat). When insulin is high, it means your metabolism loves to store fat and it prevents lipolysis (the breakdown of fat). We need insulin to be low to allow for the breakdown of fat and fat loss.
Causes of Insulin Resistance:
◉ Visceral fat (Fat around organs or fat in the abdominal area)
◉ Certain Ethnicity (Latino, African American, Native American, or Asian American)
◉ Aging (As we age we become more insulin resistant)
◉ Nutrient deficiencies (Low Mg, Vit. D, B12, increases the risk of IR )

◉ Metabolic Syndrome is an advanced case of Insulin Resistance.

Insulin resistance can cause low and high blood sugars. Patients are craving sugars because their cells are starving in the face of plenty. Eventually they will have enough insulin to break down the barricade in which the gates open and allow the glucose in the blood to go into the cells and their sugars drop. This triggers a number of symptoms like, craving sweets, lack of energy, (I need to take a nap) mental fogginess, shakiness and trouble sleeping.
This becomes a vicious cycle. Your cells are screaming eat more carbs and sugar, but all that glucose can’t go into the cells since you have insulin resistance so it gets stored as fat. You are still hungry. Your metabolism slows since the cells aren’t getting the fuel they need so they think you are starving and the body tries to conserve energy. The result is you gain weight despite eating the same amount or less.

What Is Leptin and Leptin Resistance?
Leptin is a hormone produced by fat cells that when it binds to receptors in the brain, it tells us to stop eating. The more fat that we have, the larger the signal is to tell us to stop eating. This signalling works well when we eat our ancestral diets. When we stray from this diet by eating carbs, for example, this signalling pathway breaks down, and we can have high leptin levels but the receptors in the brain are resistant, so we keep on eating. Fructose, especially HFCS (High Fructose Corn Syrup) renders the hypothalamus resistant to leptin, so we keep on eating. High TG (triglycerides) blocks leptin from passing the blood brain barrier so leptin can bind to the receptors so you’re hungry.
Eating a low CHO diet (or a low Glycemic Index diet -low GI) will reduce TG and improve leptin sensitivity. Another interesting fact is that leptins from wheat, barley, and rye binds to the leptin receptors in the brain and blocks it; therefore, leptin can’t bind, and it causes leptin resistance so, again you’re hungry. Elevated CRP which is a sign of inflammation and is commonly elevated in obese patients, binds leptin and therefore prevents it binding in the brain to tell us to stop eating. So obese patients are hungry in the face of fuel excess. Doesn’t this seem like a vicious cycle?
The low-calorie diet and low carb diet will improve leptin resistance i.e. increasing leptin sensitivity in the brain and allowing for “the leptin signal” to tell us we are no longer hungry (of course we need to listen). A very low calory diet alone will reduce leptin secretion, which will tell your brain to eat and be hungry. Haven’t we all felt this where you barely eat all day and at the end of the day you start eating and you just can’t stop? That’s Leptin at work.
